[cisco-voip] Temp Fail message on phones

Client of ours came across this issue for a batch of phones last year. Performed a factory reset, moved the phones off to a different switch port, moved them to a completely different switch, upgraded the firmware, swapped out the patch cables, checked call statistics for lost packets..... As for codec mismatches or unregistration events, CCM traces didn't show any codec mismatches (no transcoders invoked) or  gateway unregistration events or d-channel flaps during the time the problem occurred. SCCP and Q931 signaling showed the disconnect was issued from the end user, not PSTN. Eventually opened a TAC case and was told it was due to hardware failure. Client RMA'ed the IP phones and never encountered the issue again.

Started seeing this error message last week at a remote location connected via MPLS on several phones intermittently.    People are actually on a phone call and get disconnected when the message appears.  After searching the archives it looks like there are multiple possibilites for the cause of this message: Packet loss, DSP issues, Codec mismatch, MGCP gateways unregistering, etc.

Anyone have a clue what the likely cause would be when you have an H.323 gateway and calls are already connected but get dropped in the middle?
