[cisco-voip] On hook off hook IP polycom

Has anyone experienced the following, you dial a number and you make a
mistake and hit the on hook off hook button to regain dial tone but
nothing happens?

Then you have to physically remove the data cable to get dial one back.

This problem seems to be only affecting the cisco polycom's.
