[cisco-voip] What would cause DTMF not to go through

I have not tested this yet, but user says they call a number and the DTMF is
not working.  This is G711 and other ones work.  It's just this number.  Any
ideas?  Firmware on the phones is 8.4.1 and CM is 6.1.2.
