I'm pretty sure it's the gateway, we tried dialing back and forth
internally and it was fine every time.. We ran this setup but with
different hardware (1700V routers for pstn gateways and same exact
hardware revision and image on the ata186s).
The region is definitely set for G.711.
The relevant part of the config is below with ccm IP xxx'd out.. Im
hoping I just overlooked something.

I would be interested to see your gateway config. I assume you have
modem passthrough enabled everywhere. Also, is your ATA in a device pool
with a region set for G711?

We've been running a ccm setup for about 5 years now, many versions..
we've always used ata186s for faxes and modems.. We recently had a
tech refresh of the system after a lease ended and we can't get the
modems to work. Actually there is only 1 modem we care about. The one
for the security system. We've never needed to give it a dedicated pots
line and would prefer to not go that route. We are fully aware the
ata186 doesn't support modem passthrough but its always worked in the
past.
We're using PRI lines with 2811 routers for the pstn interface.
Everything is setup right, the modem does answer but the call drops
about 30 seconds later on its own. Well we think on its own, we're not
sure if its just the security company software timing out, they
obviously have very little information about their side...
Is anyone using a 2800 series router with modem passthrough combined
with ATA186 adapters? I'd love to know your config for the combination
of hardware.
