Check the ports in RTMT, see whether or not they show up as idle. If
you put a phone in a CSS that can call the ports directly, do any of
them answer? That version of UC is getting a little long in the tooth.
A quick bug search turns up bugs such as:

CSCsl13307 - CUC port locks up, can't process calls
CSCsm41655 - Unity Connection 2 stops answering calls

Upgrading, or at least an ES, would probably be a good idea.
