It would be helpful for you to post your dial-peers section.
If you had this problem once before I can't shake the feeling that  
what solved it was a
dial-peer voip blah with a fully literal destination-pattern, and  
thats why it pops up again
the same way on a different number.

No matter what, at this point, where it rings and isn't forwarding,  I  
think the problem could be
easily spotted in the dial-peers section of your show run.
