With CCM 4.2 the only Outlook dialing you get is via the Cisco TSP.  It's
not TAC supported and I don't think the doc is on cisco.com any longer but
Google should turn up some info for you.  The Click to Call "widget" you can
download from cisco.com only works with CUCM 5.1 and later.
