What type of gateway is it? If H.323 make sure iLBC is either set on the voip dial-peer or in a codec class applied to the dial-peer.

If MGCP check out http://www.cisco.com/en/US/docs/ios/12_4t/12_4t15/it_amrnb.html.

Doing some testing with iLbc. I can place a call from IP phone (7962) to IP phone 7962 using iLbC and 7962 to IP Communicator 7 and I see I am using the iLBC codec on each end of the connection. I negotiate G729 when I use a gateway to reach the PSTN. Far as I can tell all is set up correctly with my regions, locations, device pools etc. Is iLBC supported in the following path between the phone and gateway?
7962 --IP------2851-------PRI-------PSTN
Gateway is running 12.4.20T and CUCM is 6.1.1
Thanks
Steve
