[cisco-voip] Instant Fast Busy Outbound Calls

Another site, another issue.....
 
MGCP gateway.  3 ports configured on FXO for outbound calling.  Dial out
and get instant fast busy.   Not a new site; it has been working, but it
stopped.  Tried rebooting router, reconfiguring gateway, resetting
gateways, restarting mgcp.  Gateway is registered.
 
Debug shows call hitting router, but it just cycles through all ports
and gives fast busy:

Another physical layer problem?  I can't think what else; we've not
changed anything for that site.  What else might I do before driving out
there with a buttset to test from the demarc?
