We had the same issue. Wes commented on a couple of bugs. We had all 140 ports or so doing MWI....we moved back to the 10% rule. Haven't had the problem since.

I am working with TAC on this issue but wondering as to how others have their Connection Port Groups set up for MWI. Every time I make a change and reset to the Port Group this problem goes away but returns a couple of days later. I am set for 5 maximum concurrent calls with 5000ms of delay. This is a Connection 7.1.3 system with a 6.1.3b CUCM
