[cisco-voip] DMA 7.1.3 Errors

First one is a phone button template that a phone is set to use, but  
probably doesn't exist any more.

Confirm with a sql query:
select * from phonetemplate where  
pkid='d3f44f1b-080d-445c-9092-76904c0a9bf6'

If you don't get any results then you'll need to fix the phone(s) that  
reference this template.

The second error looks like a phone that had its device profile  
deleted while it was logged in.  Force the phone to logout of EM and  
you should be good to go.
