[cisco-voip] SIP Trunk provider recommendations

I'm using Broadvox for my test lab.  Got 3 concurrent calls (local calling, 2 cents LD) and 10 DIDs for around $50/month (about $60 with taxes).  I got local since my testing is mostly inbound.
