[cisco-voip] Unity 5 Caller Input and Call Handler Questions

Greetings,
 
I want to use Unity to give a caller who reaches VM to have 4 options:
 
Press 1 to go directly to a subscriber's VM
Press 2 to go directly to another subscriber's VM
Press 3 to go to either an outside number or a route point in CM
Press 4 to go to either an outside number or a route point In CM
And if no input is given, caller is sent to VM of the original called
number.
 
In looking at the Subscriber's caller input choices or in a Call
Handler's caller input choices, I'm not seeing how to do this.
 
In the Subscriber's caller input, apparently I can only set a single
Alternate Contact Number.  While I can set input numbers 1, 2, 3, to
send to an Alternate Contact Number, that number is always the same for
all 3 input numbers.  If I can change one Alternate, they all change.
 
In a Call Handler, I can't see where I can get a call back to call
manager to send it out into the world.
 
This can be done, surely.  Any suggestions to help me with what must be
bleedingly obvious....
