[cisco-voip] The agent is already LOGGED ON error

When agent tries to connect to CTI OS server, it is providing the following
failure event:
 
 eCTIOSFailureEvent

    (PeripheralID:5000 FailureCode:11 PeripheralErrorCode:0 
AgentID:32719418 EnablementMask:0x1400000 
UniqueObjectID:agent.5000.32719418 EventID:536870924 
MessageID:eControlFailureConf MessageType:38 ErrorMessage:The request 
failed because a timeout limit was exceeded. EventType:2 
FilterTarget:agent.5000.32719418 
TargetCILClientID:.1408dotNET_Sess(30180123)_EvtThd(15) 
BlockAllEvents:True)

and CTI OS client tries failover after which it is dumping the following
error continuously

Can any of you help to find out the issue. Server has a setting to force
login if agent is logged in somewhere else
