[cisco-voip] 7921 fireware update fail

I've seen this happen when the firmware on the phone is more current than what's available on the TFTP server.  The 7921 won't revert to a lower firmware revision and the update fails.

Shortly after that it says upgrade failed, I have tried this across the VPN and from a wireless access point connected to my TFTP server.
