"Cause 49 / Code B1: Quality of Service not available"

you may be running into a bit of interesting translation here.  CM can 
return code 49 for codec mismatch or transcoder allocation failure.

alternatively it looks like this is the code that is used when RSVP 
reservation is required but fails.
