[cisco-voip] Codec Mixing

This shouldn't technically require any transcoding in the form we know
it as, where the transcoder is controlled by cucm. yes, the gateway is
transcoding from mu-law to a-law, but this should get done as part of
the TDM-termination function. did you say it was working?
