Whether it's executed or not probably depends on the phone model.

In the mean time, I thought exactly what you were thinking. I created a service, enterprise subscription and am able to select it from the list. It is the only one subscribed, which I thought too, would execute that service.

No idea why both isn't working correctly for your service unless it's a bug or the "both" option doesn't apply to the services URL.
Why not add your custom URL as an actual service in CCMAdmin? Make it an Enterprise Subscription (subscribes it to all phones) and assign it to the services button and if that's a phone's only subscribed service the phone should launch it immediately.
If you want to pursue why the phone isn't showing the service currently then you'll need a packet capture from the phone to see if it's even requesting the service URL when you hit the button.

When you push the Directories button, you should see:
Missed Calls
Received Calls

Campus Directory (our custom app URL)
When you push the Services button, you get our custom app URL. Basically and XML file saying: "Additional Services & Training are available at ..."
Unfortunately, I can't seem to get both working properly.
If I set "Services Provisioning" to "External URL", I get the Services button working properly, but when I hit the Directories button, I get only the Campus Directory option. The Missed, Received, Placed calls are missing.
If I set "Services Provisioning" to "Both", the Directories button behaves like we want, i.e. all four directories appear, but the Services button displays a blank screen with title "Services" and prompt, "Select Service..."
Any ideas on how to get both working?
