Why not add an intrasite prefix by using translation patterns. Users can't call other locations based on 4 digit anymore but by 5 digits.

Less programming oriented with better set granularity may be phone export/import. Use BPS/BAT feature to export phones, modify as desired, and import.

I don't have the ability to write something from scratch so examples are welcome or suggestions of better ways to handle this type of scenario. I've never had to do something like this before and I want to keep it simple but give me plenty of room to grow. I know some people advocate 10 digit extensions with transformation and translations to allow 4 digit type dialing but something of the scope I feel may be beyond my capacity to handle.
