[cisco-voip] 50% packetloss on 6921 IP Phone connected to Cat4500

Have you come across problems with IP phones connected to Cat4500
running IOS 12.2(54)SG1?

I have packetloss, phone drops regisration and IP, restarts etc.

Moved the phone to a 2960 switch and it worked right away.

Port has CDP, PoE, Dot1x, QoS trust etc. I have tried to reset a port
to default without anything special, but still same problem.
