Inside Cisco we tend to use MSCs:

There are several tools that automatically produce these based on a textual description of the environment. Text can be generated manually or crafted from traces or CDR's.

Just wondering what kind of docs people are keeping on call flows in their systems. I'd be interested to know how you're actually capturing the call flows (viso / graphs etc) and what info you're keeping on hand. I'm trying to come up with some kind of standard I can follow but can't seem to see a whole lot around.
