[cisco-voip] 7961G Can't do conf call

I have a weird one.
 
CM ver 6.1.4.1190-3
 
2 users both with 7961G's and identical settings one can do a conf call and one can't. With the one that can't do conf call we can call out to each party then hit the confn soft key and both calls drop. On the other phone it works fine.
 
I've swapped the phones copied the profile etc.. no go.
 
Phone loads on both phones are: SCCP41.8-5-4S
