Based on this debug the provider is not sending you anything for calling party name or number. It would come through in a an IE or in a separate Facility message and Facility IE neither of which are present here. More discussions with telco required...

Looks like maybe you're getting inbound calls on an E1. Likely a PRI? In that case 'debug isdn q931' would be helpful.
Several ISDN protocols have switched to cnam format of name delivery which comes through in a subsequent facility message:
