Are you running Multicast for your MOH?  try turning it off and see if it clears the problem. Please let me know. I have a problem with Multicast doing almost the same thing. I am running SIP

About the explanation… It seems that the H323 Gateway does not like the IP Address change when you put the call on hold. The audio stream source ip address changes from your phone to the assigned MOH server, and when you release the call, it goes back from the MOH server to your phone.
The use of MTP masks this behavior to the gateway.

I would check that the Gateways IOS is updated.

Not in my case ,I can actually see the MTP resources are active and in use on my GW ,using show sccp connections .

We ran into the same problem when we upgraded to 8.6 and we use SIP.  TAC said CUCM will ask for the resource but never uses it.

MoH did work ,call only fails when trying to resume from being placed on hold. Basically the problem seems to be resolved by forcing the GW to use MTP ,this is what I don`t understand ?

Typically this happens because the hold actually failed. did the PSTN user actually hear music on hold? look for codec mismatches or misconfiguration in delivering MoH to the PSTN caller.

I have the following scenario at a client site ,call comes from PSTN via H323 GW ,I can answer the call and place the caller on hold ,when I try and resume the call ,the Telco drops the call with a “ Bearer capability not implemented” error. Any ideas what the problem could be ?
