[cisco-voip] 2911 with PVDM-3 and 3x9971s and no Video Conference Homogeneous

I had this working back in my lab at Cisco at the beginning of the year.  I
don't have that config anymore though.  I remember it was a bit finicky to
get it working right.

Try increasing the max video bit rate for video calls on the region to
something like 10000 to eliminate that as a potential issue.  The bitrate
on your conference bridge configuration is pretty high.

CallManager traces may be useful here as well to eliminate anything on that
side.
