Good thread. Gonna need some of this.
Currently, our CUCM/AD integration is using telephoneNumber attribute. Unfortunate, because we populate that with "+1-519-824-4120 x12345" it messes up the reverse contact lookup.
Because we have chosen to use UDS (to ensure similar behaviour both on and off-premise via MRA), it seems my only option is to populate the ipphone attribute with the extension and rebuild the LDAP sync relationship using ipphone instead of telephoneNumber.
UDS doesn't offer many config settings.

WorkPhone maps to telephoneNumber in AD by default
You would need something like this under the Directory section if you want to use the the OtherPhone field and leave WorkPhone the same:

<OtherPhone>ipPhone</OtherPhone>
