  <DIV><BR></DIV>A Translation Pattern or Route Pattern of 91 could do this if 
  the called party is changed to 911. Would seem strange that something like 
  this would be configured though. There are no issues where CCM would change 91 
  to 911. Either there is a user related issue as noted below or a config issue. 
  CCM traces will show you what the users dial when this problem
