You will not get this in a single report from CM. IP address
information is stored in RIS / shared memory. Only insight is
DeviceListX.asp or mmfspy from cmd prompt. DN and device description
are stored in different tables in the (normalized) SQL database.<br>

you can get dn & phone from a route plan report ( ccmadmin, route
plan, route plan report)<br>
you can then merge this information with devicelistx to match device
names to IP addresses<br>
