I only have a cisco 1811w at the site so If I need CUBE I will have to upgrade the router ugh....so is it possible to do this without CUBE? I thought I could just nat a public ip address to the gateway and cucm would be able to support the trunk ( this is my first time setting up a CUCM so I am a bit new at this)

I think native SIP trunks are supported as far back as 4.x (we're using one on 4.1) but it all depends on the feature set and visibility. For example, if you're on a privately adressed VLAN you'll need CUBE or something similar.
