In CM6.1.2 similar error was caused by username or password incorrect.
looks liek that has been fixed in 6.1.3.<br>

attempting to change hostname during upgrade, while technically
supported, rarely goes well. I suspect your database is not strated.
login to the CLI and check "utils service list". Is database running?<br>

6.1.3, and we used a DMA file to import the database from CUCM 4.2.3

only change we did when importing the DMA file was to change the
