when I tried this last year (this was on 5.2), it would not upgrade with
the hitless-upgrade command. After rebooting the standby module with
reboot-standby it did not come up due to a version mismatch with the
active management module.

I am still waiting for the first successful hitless upgrade,
unfortunately this is not always possible even within a version tree.
This is a citation from NetIronUnified_05300b_UpgradeGuide.pdf, page 29:

Hitless OS Upgrade support limitations
Depending on the software version, Hitless OS Upgrade has the following
limitations:
• Hitless OS Upgrade is not supported for R05.0.00, R05.0.00a, R5.2.00b,
R05.0.00c, or R5.2.00d, R5.2.00f, or R5.3.00a and R5.3.00b.
• Hitless OS Upgrade is not supported for releases prior to R05.1.00d.
This includes R05.1.00, R05.1.00b, R05.1.00c and R05.1.00d.

By the way, hitless switchover always works well.
