Hi Keith,
Sorry, I don't know where you could find those scarce "4500s".  J.W.Miller
components are getting very hard to find any more.  20 years ago, they were
readilly available from many parts sources.  However, Dan's Parts online
catalog advertises similar Millen ceramic adjustable 3/8" dia. forms at the
address below.

It may be an alternative source.   Hope this helps.
