Isn't this just a plastic sleeve ? If so how about a piece of the
smaller coax.....take a 4" section and slide the outer plastic
off by 1/4" or so to allow room for the nut. Maybe a piece of
shrink tube on the end of small coax would work. Many times
I've used double sided tape or a tape loop on the end of a
screwdriver for one or two.

Maybe this is some fancier tool supplied later on by Heathkit
but I remember a ribbed red plastic tube ?

According to the DESCO website, one of their distributors is Allied
Electronics.  But a search of the on-line Allied catalog does not turn it
up, either under "DESCO 00200" nor under "nut starter".
