WTB: Metering Jack for HW-29A Sixer

The metering jack for the Sixer I am rebuilding
literally fell apart in my hands when I removed it
from the chassis.  At first I thought it was simply a
1/4" stereo phone jack but such is not the case.

It is Heath part number 436-6 described as a 3 circuit
phone jack.  When you partially insert a standard 1/4"
phone plug into the jack you meter relative output on
the first contact.  Pushing the plug in all the way
activates the second contact which is switched and
opens the cathode connection to the PA for current
measurement.

I'm sure the same jack is used in the Twoer, Tener and
CB-1. Anyone have one laying around from a junker?
