WTB: Modification instructions for DX-100

Good day to the list.

I'm looking for a copy of page 3 of the Heathkit MK-3 Factory 
modification instructions.  This is the page that has the template and 
drilling specs for mounting the "new" loading capacitor.

Any help would be appreciated.
