[j-nsp] M20 FPC detaching/going offline for no apparent reason

Has anyone seen this behavior before?

M20 (5.6R2.4) FPC3 detached/went offline.

Ok, replaced FPC.

A week later, the replacement FPC does the same.
These are the only log messages:

Looking at a show chassis fpc detail shows it is down:

Reseating the slot, it comes right back up:

Any ideas?

String of bad FPC's?
Software issue?
Chassis/backplane issue?
