[j-nsp] EX-4200 VC, unable to lock config on fpc

Hey everyone,

We recently upgraded a fistful of EX-4200s to 9.3S1.6 and have
acquired a new issue.  At a certain point, trying to commit will give
an error:

fpc3:
error: configuration database modified
fpc0:
error: remote lock-configuration failed on fpc3

Once this hits, you can't commit any changes.  Attempting to "request
session member 3" results in rlogin dying as soon as the session is
established with:

/usr/libexec/ld-elf.so.1: Cannot open "/usr/lib/libutil.so.5"

However, using "file list detail", I can confirm that the shared
library is present.  Further, the size and modification details match
working members.

I don't think it's a flash disk problem.  First, rebooting the
affected fpc will clear the condition.  Second, this has now struck
three different VCs.

Anyone else ran into this?
