[j-nsp] Code upgrade to 10.4R4.5

I'm sure someone from Juniper will say that going forward like this "isn't supported".

I recommend skipping the validate step and making sure you have good oob.  You can also try to find something where the upgrade path is supported, in theory you need to jump to 10.0 before 10.4 but i've found that if you use the jinstall that's not needed.
