[outages] Cogent - Latency/Packet Loss out of SFO

Further info from Cogent:

Cogent reports this was caused due to a NTP amplification attack on 
another customer.

The issue came back at around 9:15AM PST for about 10 minutes, and again 
at 9:35PM PST for about 5 minutes.  Cogent says these were due to the 
same customer being attacked.
