[outages] Lighpath down completely NYC / NJ

We have a customer with Lightpath IP in Morristown, NJ that's down (and has
been down since about 4:05ET.)  We have some point to point Ethernet
circuits that don't seem to be affected so I'm assuming it's just on the IP
side of things.
