Hi Marcin,
About M flag, the doc says: "This flag appears only for mandatory FPGA upgrades; it does not appear for optional FPGA upgrades.".
So M flag only appears if the upgrade is needed for the card to start. However you are still expected to update the FPGA even if it's "optional" (according to the doc) when you upgrade your SEOS.
So what about updating the FPGA ? :)
By the way, here we also had one 10ge-4 crash/reboot with 11.1.2.5 (FPGA was up to date).
