Here is an article talking about how companies like TekStar operate and make their money. It is a bit old (2007) but apparently still applicable.

Seems to be that TekStar is a carrier in that NPA/NXX and they managed
to figure out a way to charge outrageous carrier access charges to
support among other things a free conference calling service (most
likely it's the other way around with what is supporting what).
Because of this Level3 and Bandwidth are both not reliably completing
calls there. In the case of Bandwidth they're just refusing the calls
all together, while Level 3 (at least on the Vector platform) will let
you complete a call there every once in a while.

rural areas. There is currently no ETA for a fix. Level3 has a limited
amount of capacity in these particular areas and at this time we will

from this type of activities and the cost that goes along with it.
