[VoiceOps] Polycom device.prov.* flash parameters

Here's what I use. The trick is you have to set all the *.set parameters to "1":

device.prov.serverName.set="1"		device.prov.serverName="ceres.example.com"

device.prov.password.set="1"		device.prov.password="somepass"
