[VoiceOps] Origination/Termination Vendor for Puerto Rico

Vitelity.com does both.  Despite their downtime yesterday due to XO and a
power outage this morning, their uptime was 99.834% between 11/7/09 and
12/31/09, and 99.780% since Jan 1 this year.  For the price, I really can't
complain.
