"Firm bidirectional acknowledgment" is already required in an 
authenticated REGISTER sequence:

1. ---------- REGISTER ---------->
2. <-------- 401 challenge -------
4. ------- REGISTER + digest ---->
5. <---------- 200 OK ------------

Unless this happens, an attempt cannot be regarded to "succeed."
