[VoiceOps] Toll free SIP termination

Thought I'd provide a follow-up for this.
I've been using Alcazar for a few weeks now and have no complaints.
They're about 40ms away from me via Global Crossing. No bells and
whistles, but good voice quality and reliability. Our monitoring
system did report a short interruption in the middle of the night last
week, was probably just some sort of maintenance.
I'm using my current toll termination providers as backup if Alcazar
doesn't set the call up within 1.5 sec.
