There is a subsubversion number, like in "12.0(16)S3 -> 12.0(16)S8".

Usually you have no additional digits, like in "12.0(16)S".

There does not seem to be a rebuild for 18-ST, but there are a number
for 19-ST (12.0(19)ST1 and 12.0(19)ST2, the latter having been released
to CCO this Sunday).

There are some docs on www.cisco.com that explain this, but I do not
have any URLs handy. Try to search for "IOS naming rebuild" or something
like this.
