I think that could be your problem.

You are most likely hitting CSCei63851:

AS5300: MICA modems randomly marked as BAD in any version
        after IOS 12.3(6).

The workaround is to downgrade to IOS 12.3(6)d or anything 
before it.

At the time I had this issue with one of my AS5300's, 12.3
(6)d was DF. 12.3(5)f saved my life. The MICA modems were 
back in shape.

Also, be sure to be running MICA portware 2.9.5.0.
