Try manually binding your signalling protocol to the inside interface. 

For H.323 use the interface command:

h323-gateway voip bind srcaddr. 



For mgcp use the commands:

mgcp bind control source-interface

mgcp bind media source-interface



FYI, apparently you can't use dsp resources (for
conferencing/transcoding) that reside in the same router that terminates
the VPN  tunnel.  I tried this for a customer and the sccp traffic would
originate from the outside interface (even when using the sccp local
command).  I had a case open for over a month before TAC told me it
wasn't supported.
