[cisco-voip] Unregistered remote handset in CM -> forward call out PSTN

With CallManager 4.2(1) you have new fields on the DN for:
Forward Unregistered Internal & Forward Unregistered External
I'm not sure what flavors of CCM 5.x have those fields or whether this
can be accomplished in 4.1 or below via some type of HG config.
